Use this form to notify the Achievement Scheme Review Committee of the details of your Examiner/Instructor Workshop so your workshop can be added to the Achievement Scheme website. An online booking form will also be setup so people can book on to your workshop. Notifications of bookings will be sent to the lead organiser.
